When you come for your appointment please remember to bring the following:
- a referral letter from your GP and any relevant medical documents e.g. MRIs, prescription sheets, medical reports would be helpful
- Medicare card, DVA card, pension card
- your Private Hospital Insurance information
- reports, X-rays, MRI’s, CT scans etc. and any other relevant information.
What if I need to cancel?
We understand that sometimes things come up and you won’t be able to make your appointment. To cancel an appointment, please phone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
We also understand that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
