Repeat Prescriptions or Action Plans
To request a repeat prescription or action plan, please complete the form via the link below.
Please remember that a regular review is required for your child and if they have not been seen by the doctor in the last 1-2 years.
The doctor may say it is not appropriate to write a new script or plan until your child has been reviewed.
Another appointment will be required if your child’s condition has changed significantly.
Your prescription and/or action plan will be sent to you within 2 weeks of receipt of this form.
All repeat requests incur a $5.00 fee and you will be contacted by reception for payment prior to completion.
